NEW PHYSICAL THERAPY PATIENTS

A quick, 2-part process: After scheduling your initial evaluation with our front desk or online, you will receive an email link with an electronic intake form for your health history which needs to be filled out prior to your first appointment. In addition to this electronic form, please click the link below complete your registration process through our Mindbody software. Please note that the the medical intake link you receive via email is a separate program. If you have any issues with either of these e- forms, please arrive at least 15 minutes early to your scheduled appointment to fill out paper copies on site.
